Volunteer Reports

Monthly activity reports are very important to your client because knowing what you are doing and what your concerns are allows us to work as a team for the best interest of your client. Reports are also important because:

  • GSI is accountable to Tarrant County Probate Court and various funders for all activities with and for clients – including the actions of volunteers.
  • The GSI case manager needs to know what is happening in your client’s life in order to help the client and help you.
  • Your report becomes part of the client’s record and helps give an accurate picture of client's situation.

Due Date: The 4th of each month for the previous month’s activities.

Report Formats:  We at Guardianship Services want to make reporting as easy as possible; therefore, we offer a number of ways to send us reports.  Choose the one that is best for you.

The Alzheimer's Association has organized a new program, REACH, which provides resources for caregivers. Call 1-800-272-3900 to learn more about the program.

Are you experiencing problems obtaining services for older adults and people with disabilities? Call the Aging and Disability Resource Center, 1-888-730-2372. You are also welcome to visit the center from 9 a.m.-5 p.m. Monday-Friday at 1300 Circle Drive, Fort Worth, Texas 76119.
